You will need:
this screencap
texture 1 (by lookslikerain)
texture 2 (by innocent_lexys)
font 1 (Black Chancery)
font 2 (Last King Quest)
1. Fill a 100x100 canvas with #236B9A solid colour
2. Take texture 1 and edit it to look like this and set to Screen opacity 100:

3. Take texture 1 and edit it to look like this this and set to Soft Light opactity 100:

4. Create a text layer that looks like this using font 1. I had two layers, the lighter one underneath, darker one on top and shifted a couple of pixels up and to the right. The black bits are so you can see the font properly btw:

5. Take the screencap and crop and resize and cut out Sansa to get this base (minor the black bits - that's just so you can see where the cut-out is). I did some minor adjustments to the colour/saturation here:

6. She looks a bit dark so duplicate the Sansa layer and set to Screen opacity 50, and then because she looks a bit too vibrant lower the saturation slightly:

7. Her face looks a bit dark so duplicate the second Sansa layer and erase everything except her face, then set to Screen opacity 50:

8. Take texture 2 and edit it to look like this. The blob in the middle is where I’ve erased where her face is using an eraser on opacity about 50. Set to Soft Light opacity 100:

9. Create a text layer that looks like this using font 2. Again I had two layers, this time with the lighter layer on top and shifted up by a couple of pixels, and again the black bits are just so you can see the font:

10. The whole thing is a bit too light and bright so create a new layer from visible and adjust the curves to get something like this:
